The Surgical Correction of Atrial Septal Defect with Both Primum and Secundum Type

Abstract
The incidence of the occurrence of ostium primum and ostium secundum defect of atrial septal defect in the same patient is very low.
The statistics indicate that the association of these two defects occurred in 3 cases (4.8%) out of 62 cases of atrial septal defect.
A case of atrial septal defect which had association of primum and secundum type defect is reported.
This anomaly was found in a 34 year old housewife who was successfully treated by open heart surgery.
With the use of the extracorporeal circulation the secundum defect was closed with sutures and the primum defect was closed with Teflon patch. There was a small cleft in mitral valve cusp which was not closed since this would aggravate mitral insufficiency.
Although the systolic murmur of mital insufficiency remained postoperatively, the patient¢¥s condition was markedly improved with the closure of larger defect of the atrial septum.
It is our belief that one should be very careful not to injure the atrioventricular conduction system especially when he closes the atrial septal defect of primum type.
